Over here in Europe...

We tend to use www.eurocircuits.com for our prototypes.
They can manage 3 day and are very cheap.

The FR4 looks a bit on the cheap side but for our prototypes having a manufactured board is far better than anything we can etch ourselves &the cost is usually less than it will cost for tooling alone at our normal suppliers.
