In Standard IPC-2221A section 8.3.1 it states "Component leads, jumper wires 
and other leads shall be mounted such that there is only one lead in any hole 
except as specified in 8.2.13." I have been in the industry a long time and 
don't recall seeing any designs that violate this rule so it seems to be 
honored. My question is what are the problems that result if more that one 
lead is inserted per hole?
