The most common problem I've seen in using footprints created with mixed units of measure is the way in which the CAD system handles them when automatically converting to a common unit of measure.  More specifically, when a CAD system converts from one unit of measure to another, there may be an accumulation of rounded-off dimensions during the process, that may result in footprint patterns with slightly distorted pad locations.

The problem is common to fine pitch components and components with unusual lead spacing dimensions.  Just be sure to dimensionally verify your footprints prior to starting the routing phase.
