I have submitted boards for UL review for both Industrial Controls and Food
Service, the document you need to review is UL #508  sec. 48A  ( clearance
and creepage distances)   64A (if you do not meet 48A) and 57C (if you use
conformal coating).

The last product I had approved cost ~ $1700 and took about 10 weeks for the
UL rating.
