Craig,
I've participated in a number of design reviews using WebEx, (I believe GoToMeeting is similar). We've had people from different locations worldwide participating and it has worked well. 

I generally send out a complete set of gerbers, netlist, DRC report, etc a day or two before the design review. Once everyone calls in and connects to the meeting I "share" my desktop which projects my screen onto everyone's display and we do the review.  

I also participate in on site reviews with some customers using a projector, but these are limited to local clients and burn a lot of time.

In the "old" days we used to gather in a conference room and look over E size plots. Not a very good process, but I guess it was better than nothing.

Yes, I have checklists and we have a (somewhat) formal design review process.

But what tools are being used out there?

When your 2nd (or 3rd) set of eyes look over your work, are they literally over your shoulder going net-by-net at the PCB or CAM files?
Are they looking at 1:1 or x:1 paper checkplots or tranparencies?
Anybody booking a conference space and using a projector?
If Dept. heads are signing off, what are they using to scrutinize the work?
What about file sharing and IM-ing, anybody attempting that?

I hope this better conveys what I was striving for.
