Jeff,
Yes we have seen that too on a single side,
also with insertion machines, the lead can catch
and rip up the pad up too.
But if the pad is reasonable in size to hole,
it should limit the problem, (except for the gorillas)
they always pose a problem.

I'm going waaaay back here but it sounds to me like there
may be an issue with the size of the great apes on the factory
floor.  I recall having many problems on a paper-epoxy
single sided board where handling post assembly and before
test was causing components to be pushed through and so
lifting the land, track, solder joint and everything.  These
were not small pads, either.

One cure for that was simply plating the hole, while not the
most graceful solution the added adhesion vastly reduced the
frequency and severity of damage.  What I can't recall is if
we tried simply switching to FR4.

Yikes, I'm dating myself...  Just guessing someone else in that
decision chain is my age...
