Carol,
There are numerous PCB Fabricators with cavity build experience.
I would recommend two, in the US try Merix, in Asia try  Topsearch.
I have had good luck and relationships with both of these large scale
manufacturers.
For smaller scale there are lots of houses in the Silicon Valley still that
specialize in small quantity exotic builds.
Try Steamline Circuits in San Jose.
If you ask nice any house will likely be glad to  do prototype
quantities(read this as cavity builds equal high margins for  fab).
