At my previous company, I was responsible for
implementing ISO-9001. One of the more amusing
moments was publishing a procedure on writing
procedures. Keep in mind that ISO won't really
tell you anything about how to do what you do.
The whole thing tells you how to *account* for
what you do. So, I had a cover page with title,
revision, author, author's super, and me quality
on sign-off right under revision, with date.
Title, document number number (if needed), revision
level and date are in the header for each page.
Page 2 was table of contents.
Page 3 was revision history.
Page 4 and beyond was Scope, Purpose, Definitions,
Departments/persons responsible for doing procedure,
pertinent standards. Then the Procedure itself.
There's more to it than this, but that is a basic
outline I used.
