This is how we are doing it at Inter-Tel.

Top side fiducials
Top layer   ----   copper circle 40 mils dia
Clearance "Void"  on all other layers 80 mils dia

Bottom side fiducials
Bottom layer  ----  copper circle 40 mils dia
Clearance "Void" on all other layers 80 mils dia

We place at least three global fiducials on a PCB and more if the lead pitch
falls below 50 mils for a component.  These fine pitch components get one or two
of their own called a local area fiducial.  The local area fiducials can be
located inside the component outline.  We also document the fiducials XY
location on a table on the assembly drawing.

        Would anyone Happen to know the IPC Specification # for Fiducials, or
Would anyone have any recommendations on Shape and size of Fiducials.
